At Usteleneurology, we offer a wide range of services to diagnose, treat, and manage neurological conditions, we can also provide a second opinion about your condition ( after review of your records and a consultation with you) including headaches. migraines seizure disorder/ epilepsy, Parkinson's disease, Dementias, post CVA ( chronic) conditions, Stable MS patients chronic neuropathic conditions such as peripheral neuropathy, diabetic neuropathy, ALS, etc because of limitations on physical exam imposed by teleneurology services acute neurological conditions and conditions that require a thorough neurological exam are not covered with our services, ( if you have a question about your condition specifically whether we will be able to provide services to you please contact us with your condition via email prior to setting up an appointment) we can review and or order diagnostic tests, medication management, and referrals to other specialists as needed.

What is telehealth?
Telehealth is a way to visit with healthcare providers, such as your doctor or nurse practitioner.
You can talk to your provider from any place, including your home. You don’t go to a clinic or hospital.
How do I use telehealth?
· You talk to your provider by phone, computer, or tablet.
· Video is used, so you and your provider can see each other.
How does telehealth help me?
· You don’t have to go to a clinic or hospital to see your provider.
· You won’t risk getting sick from other people.
Can telehealth be bad for me?
· You and your provider won’t be in the same room, so it may feel different than an office visit.
· Your provider may make a mistake because they cannot examine you as closely as at an office visit.
· Your provider may decide you still need an office visit.
· Technical problems may interrupt or stop your visit before you are done.
Will my telehealth visit be private?
· We will not record visits with your provider.
· If people are close to you, they may hear something you did not want them to know. You should be in a private place, so other people cannot hear you.
· Your provider will tell you if someone else from their office can hear or see you.
· We use telehealth technology that is designed to protect your privacy.
· If you use the Internet for telehealth, use a network that is private and secure.
· There is a very small chance that someone could use technology to hear or see your telehealth visit.
What if I want an office visit, not a telehealth visit?
For now, all visits are by telehealth. You cannot schedule an office with our provider, he is only providing telehealth services.
What if I try telehealth and don’t like it?
· You can stop using telehealth any time, even during a telehealth visit.
· If you decide you do not want to use telehealth again:
o Please inform us you want to stop, during a visit OR
o Cancel any future appointments with the provider
o It will be as if you never signed this form.
How much does a telehealth visit cost?
· What you pay is clearly indicated in the fee schedule.
· This is a cash pay only service not covered through any insurance, even if you carry insurance we are not taking any insurance as form of payment at this time.
Who keeps records of my visit/s?
We keep notes of your records on a HIPPA compliant EMR system
Can other providers have access to my records?
We do not provide records to any outside provider.
How can I get my records?
We can only provide records to the patient or designated family directly
Is there a fee for obtaining records?
There is a 1 dollar per page charge for medical records
How do I request records?
Email a request for all or specific records you are requesting.
How do I make payment for records requested?
We will send you a total charge and a link for payment to be made
How will I get my records?
Records will have to be mailed to you via regular mail
What is USteleneurology membership?
You can become a monthly member and have access to up to 2 visits per month.
You can talk to your provider from any place, including your home.
How does membership work?
You must be seen at least once as a consult or a second opinion for an initial visit and if you want to consider ongoing management of more than an occasional follow up or more than one a month visit membership offers a cheaper way of accessing the provider.
Are there any limits to telehealth visits for a member?
Membership visits are limited to a maximum of 2 15 minute follow up visits a month.
Can I see the provider for additional visits if I am a member?
If you need more than 2 visits per month you can always see the provider and pay the additional follow up visit charges to set any additional appointments .
Will my telehealth visit cover general medical diagnosis or questions?
We will not cover any general medical questions or concerns during your visits with our provider. The visit/s will be focused on your neurological condition/s.
How do I cancel my membership?
Please send us a request for cancellation via email. We will cancel your membership beginning of the following month.
Can I get a refund of payment already made?
No refunds are issued for used or unused services for any prior payments already made.
